Focus on queen cells, especially along the bottom of frames and on frame edges. Swarm cells are typically on the bottom bars, while supersedure cells are on the face of comb. Also check for congestion, lack of space, and backfilled brood nest (honey in brood area).

Do I need to see the queen during every inspection?

No. Finding fresh eggs (standing upright in cells) is a reliable indicator the queen was present within the last 3 days. Searching for the queen adds time and increases risk of accidentally injuring her. Only search for the queen when you need to mark her or if you suspect queenlessness.
